in my past experience, sometimes they use concealer on scars, but usually they did not. they usually only put a light foundation on me, and concealer under my eyes. i think a few times i asked them to conceal scars i thought were very noticeable. i think it could depend on how different the colouring of the skin is in the area of the scars? my acne scars, though they are much darker than the rest of my skin, are not very deep. other people may have a different experience.
when you go for your appointment, let the makeup artist know you are having it done for grad photos, so that they can choose your foundation and other products accordingly.
i think it isn't necessarily the quantity applied, but the application technique, or the texture of products and suitability for the skin type that makes makeup look 'caked on'. i'd probably suggest that you explain to them that you don't usually wear much makeup, and that you want something that looks natural (if that's what you want), but will also photograph well.
