Breakouts - what is the culprit?

I have been getting some nasty breakouts recently and I am trying to figure out what causes it. I am eliminating the products I use one by one and I have now come to my beloved Prep+Prime face. Does anyone know if it blocks the pores or causes any other reactions? I have combination skin that isn't particularly sensitive. So far my conclusion is that the best thing for my skin is to use a moisturizer after cleansing and leave it at that but that doesn't work for the days when I need to leave the house!

Possibly try another brand of Primer? i've never used Prep and Prime personally so I couldn't tell you. I use Perfecting Base by ME ME ME and never had any problems with it.

Could a change of diet or maybe a slightly unbalanced diet have caused it? It might be a long shot but it's worth a try?
