Hey everyone! I have a video tutorial for this if you're interested.
Anyway, I used:
Carbon by MAC
Blow by Medusa's Makeup
Yves Saint Laurent Eyeliner
custom made lashes/flowers/etc.

It was fun/a pain in the butt

how did you make the lashes?

Thanks so much everyone!
For the lashes, I just cut out pieces of paper and curled them a little bit around a pencil. Then I just used eyelash glue to glue them to the band of the false lashes. It was six individual pieces of paper for each--I originally tried using one piece shaped kind of like that but it wasn't flexible enough.
