Well, not really... alcohol and hygiene takes care of a lot, but of course there is always a risk of a stubborn bug hanging on. Also, you may need to sanitize larger areas than you think in order for it to be effective - I am thinking now not only of lipsticks, but as you mention in schools, hospitals etc you have to sanitize virtually every surface in sight to really minimize the risk of infection (I work in a hospital and whenever a patient is discharged for example, the bed, table, cupboard, doorhandles, wash basin, everything is wiped down with ample amounts of surface disinfectant). It isn't always possible in those settings to be as septic or hygienic as needed to keep every bug out, even if alcohol could get rid of everything (which I know it can't, not always).
As far as makeup goes: When I sanitize lipsticks before trying them on, I always dip the entire stick in the alcohol, not just the tip, and make sure I leave it in for a little while to make sure as many germs as possible die.
I could go on a rant now about over-prescription of antibiotics, but this is already too much off topic so I'll stop.
